As a courtesy, when you arrive at our office a staff member will gladly assist you with contacting your insurance carrier to obtain your coverage and benefits information. Each patient's coverage can be different. It is ultimately the patient's responsibility to know and understand their insurance coverage. There should be a toll-free number located on the back of your insurance card to call and obtain your benefits coverage for outpatient physical therapy.
We gladly accept cash, check, money orders, visa, MasterCard, and American Express. See insurance information above for a listing of insurance companies with whom we are preferred providers.
If you plan to have us submit your bill to your insurance carrier for payment you will need a prescription from a licensed physician. If you are personally paying for your treatment a prescription is generally not necessary. However, if the treating physical therapist feels it is indicated, you will be asked to see your physician for an appropriate diagnostic workup.
Yes, a prescription is transferable between states.
Yes, prescriptions are for a particular therapy, not for a specific facility.

Your first appointment will generally last an hour. After your initial visit, your follow up sessions will last from 45 minutes to one hour depending on your needs.
Please wear loose, comfortable clothing so that the therapist will be able to properly evaluate your current condition. Athletic or closed-toe shoes should also be worn.
